Application

The process begins by submitting an application on-line through the company's intranet or the Foundation's website.  Complete the form on-line, electronically submit the form, and then print a copy of the completed form.  This printed copy should be forwarded to your volunteer organization to have its representative complete the organization's verification and signature portion of the form.  After completing the application form, the organization will send it to the HCR ManorCare Foundation.  Applications are only eligible for consideration after both the on-line application has been submitted and the volunteer organization verification has been received.  If you are unable to submit an application on-line, please contact the Foundation at 419-252-5902 for guidance.  The annual application deadline is January 15th.
